PRIVACY POLICY and CONSENT FORM

 

The Insight Studio Asd association based in Bologna (BO) Via Francesco Petrarca 43 / A (hereinafter referred to as the "Association"), as OWNER OF THE PROCESSING OF PERSONAL DATA,  

 

pursuant to art. 12 of Regulation 679/2016 (European regulation on privacy), informs that:

 

1. the data you provide, through the forms to which this is attached, will be processed for the management of the institutional, organizational and technical activity of the Association;  

2. the processing, mandatory for the performance of the service, will be carried out using paper supports and / or electronic tools, with the use of all security measures to guarantee the confidentiality of personal data and to avoid undue access by non-subjects. authorized and in compliance with the regulations in force;  

3. personal data will be processed only by subjects authorized by us for the performance of the tasks required to satisfy the requested service;

4. the data may be disclosed to the following subjects, not belonging to our Association, such as promoters, insurance companies and subjects to whom it should become necessary to protect the interests of the Association or of the participants;

5. Sensitive data relating to the state of health, whether drawn up by specialized medical centers and / or by doctors in assessing the suitability of the specific practice, will be processed in compliance with the general authorizations of the Guarantor for the protection of personal data and will not be subject to of diffusion. Such data may only be brought to the attention and / or processed exclusively by the persons in charge of the processing and / or by the organizers for the sole purpose of evaluating their correspondence to the regulatory requirements and to the protection of the health of the member;

6. the subscriber is granted all the rights provided for by Regulation 679/2016 as an interested party, including:  

• has the right to obtain confirmation of the existence or not of personal data concerning him, even if not yet recorded, and their communication in an intelligible form;

•   has the right to obtain the indication:  

1.1 of the origin of personal data;

1.2 of the purposes and methods of treatment;  

1.3 of the logic applied in case of treatment carried out with the aid of electronic instruments;

1.4 of the identification details of the owner, of the managers and of the designated representative; 1.5 of the subjects or categories of subjects to whom the personal data may be communicated or who can learn about them as appointed representative in the territory of the state, managers or agents;  

• has the right to obtain:  

a) updating, rectification or, when interested, integration of data;  

b) the cancellation, transformation into anonymous form or blocking of data processed in violation of the law, including those of which it is not necessary to keep it in relation to the purposes for which the data were collected or subsequently processed;

 c) the attestation that the operations referred to in letters a. and b. have been brought to the attention, also as regards their content, of those to whom the data have been communicated or disseminated, except in the case in which this fulfillment proves impossible or involves the use of means manifesting the disproportionate to the protected right;

the interested party has the right to object, in whole or in part:

 a) to the processing of personal data concerning him for the purpose of sending advertising material, or for carrying out market research, or commercial communication.
